Position Overview: As a Palliative/Hospice Nurse Practitioner at Salus Hospice, you will work closely with a physician and interdisciplinary team to provide compassionate, patient-centered care. You'll help ensure hospice patients meet the face-to-face visit requirements and will support the team in delivering exceptional care to patients and their families. Key Responsibilities: * Conduct face-to-face patient visits to evaluate continued hospice eligibility in line with new regulations. * Collaborate with physicians and the interdisciplinary team to share clinical findings and adjust care plans. * Perform bedside evaluations, creating comprehensive care plans tailored to the patient’s needs. * Provide direct hospice care, supporting both the patient and family through sensitive and crucial moments. * Document all findings per CMS guidelines, ensuring accurate compliance. * Consult with supervising physicians and collaborate with healthcare providers within and outside of Salus Hospice. What We’re Looking For: * 2+ years of recent experience in hospice, or palliative care * Expertise in providing care to patients with end-stage disease. * Master’s degree in Nursing preferred. * Active Nurse Practitioner certification in California. * California state licensure to practice as a Nurse Practitioner. * Willingness to travel within a large geographic area to visit patients. * Must be willing to work in West LA and South Bay areas Qualifications: * Experience in delivering high-quality, compassionate hospice care. * Deep understanding of end-of-life care and regulations. * Ability to follow Standardized Procedures based on guidelines established by the Board of Registered Nursing Practice Act.
