Investigator - Administration/Quality Assurance
Mozaic (a chapter of Arc NY) is looking for a full-time Agency Investigator for Incidents as required by OPWDD 624 and 625 Regulations across several counties. Exercises discretion in handling confidential information, including medical, financial, and personal information for service recipients, staff, and outside entities.
Investigator Duties and Responsibilities:
The Investigator handles investigations involving all individuals receiving services under OPWDD, including those served by Mozaic and any partner organizations under a Management Services Agreement. They ensure the safety, well-being, and confidentiality of everyone involved throughout the process.
The Investigator works closely with the Administrator of Quality Assurance (QA) or their designee to identify and report any additional allegations to the Justice Center and/or OPWDD as needed. After completing interviews and reviewing all relevant evidence, the Investigator prepares written Preliminary Findings.
A full report is completed for each case within required timelines and shared with the Administrator of QA, Senior VP of Administration, and the Incident Review Committee. The Investigator may attend committee meetings to answer questions and clarify findings. They are also responsible for securing evidence, making necessary revisions to reports, and completing required documentation under Jonathan's Law and OPWDD guidelines.
Investigator Qualifications:
High school diploma or equivalent. Must have a strong attention to detail, excellent verbal, written, problem solving skills and ability to work independently. Experience in the field of intellectual/developmental disabilities is desired. Must have a valid New York State driver's license that is acceptable to the insurance carrier. Education and experience requirements may be waived exclusively by the Sr VP of Administration.

Full Time: 37.5 Hours per week
