Job Description:
Schedule is 7-on/7-off Nights
GENERAL OBJECTIVES:
Prepares and dispenses medication orders per physician request.
Maintains medication stock in patient care areas.
Ensures safe, appropriate, cost-effective medication therapies for patients according to policies, procedures and protocols.
PRIMARY DUTIES AND RESPONSIBILITIES
1. Interprets medication orders; accurately transcribes to and maintains computerized patient medication profiles.
2. Accurately compounds and dispenses pharmaceuticals, including IV admixture
preparations, chemotherapy, and parenteral nutrition products.
3. Complies with all applicable federal, state and local laws and regulations. Issues controlled substances to patient care area and maintains records as required by law.
4. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
5. Performs pharmokinetic drug dosing per hospital protocol.
6. Reviews/interprets C&S data for antibiotic appropriateness, recommending changes as needed.
7. Accurately reads, extracts, and interprets patient chart information.
8. Accurately detects and reports adverse drug reactions.
9. Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
10. Provides clinical consultation and clarification to practitioners. Suggests appropriate cost-effective therapeutic alternatives to medical staff, as needed.
11. Provides drug information to patients and families as needed.
12. Accurately documents all clinical activities and interventions
13. Participates in the quality improvement and medication review activities of the department. 14. Collects data; conducts quality monitors and inspections; maintains logs, records and other documentation as assigned.
15. Participates in the development and presentation of orientation, education and training programs to the pharmacy, nursing, medical and other staffs, as needed.
16. Member of Code Response Team.
QUALIFICATIONS
Education:
Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree.
Current license to practice pharmacy in the state of Colorado
BLS (American Heart Association)
ACLS (American Heart Association)
Flu Vaccine REQUIRED - NO EXEMPTIONS ACCEPTED - required Oct 1st through April 1.
